Hudson Headwaters Health Network, a federally qualified network of health centers, sought to enhance community access to healthcare and broaden the range of services offered. By establishing a new health center alongside a PACE (Program of All-Inclusive Care for the Elderly) facility, the network has significantly improved access to services. The health center provides comprehensive dental, primary care, laboratory, and behavioral health services. Its planning model supports team-based care in a collaborative environment, maximizing visual management of exam rooms. The PACE facility delivers comprehensive medical and social services for elderly patients requiring advanced care as an alternative to a full-time nursing home or assisted living placement.
 
As the design phase commenced, the COVID-19 pandemic reshaped the landscape of healthcare delivery, necessitating adaptations to prioritize health, safety, and pandemic readiness. The design process included testing multiple layouts and configurations to determine the optimal arrangement for individuals navigating the facility. Leveraging advanced technology and thoughtful design, SMRT streamlined movement within the facility, optimizing space and ensuring continuous operational flow across various programmatic areas while adhering to strict regulatory requirements. The design was reinforced with a comprehensive wayfinding approach with clear pathways and visual cues, including colors, art, and branding. With a key goal to foster interconnectedness within the facility, the design makes navigation intuitive and enhances the overall user experience.

This project embodies a holistic approach to healthcare design and delivery, ensuring the facility serves its community effectively now and into the future. Strategic furniture selections create zones for privacy without walls, enabling future adaptability as the community’s needs evolve.

Exam rooms maximize patient and staff communication. Mobile and adjustable computer tables allow the provider to move to the patient.
The participant activity space is designed with three flexible zones within a single area, enabling multiple activities to occur simultaneously while optimizing space usage and staff supervision.
